Lamictal: Precision Mood Stabilization for Bipolar Disorder

Lamictal (lamotrigine) is a first-line anticonvulsant medication approved by the FDA for the maintenance treatment of bipolar I disorder to delay the time to occurrence of mood episodes. It is a phenyltriazine-class agent that functions as a voltage-gated sodium channel blocker, modulating the release of glutamate and aspartate. Its distinct mechanism offers a favorable side effect profile and is particularly noted for its efficacy in preventing depressive episodes. This medication requires careful titration to mitigate the risk of serious dermatological reactions.

Common use

Lamictal is primarily indicated for the maintenance treatment of bipolar I disorder in adults to delay the time to occurrence of mood episodes (depression, mania, hypomania, mixed episodes). It is also approved for the treatment of partial seizures, generalized seizures of Lennox-Gastaut syndrome, and primary generalized tonic-clonic seizures in adults and pediatric patients. In clinical practice, it is also sometimes used off-label as adjunctive therapy in major depressive disorder and certain other neurological conditions, though such use should be guided by a specialist.

Dosage and direction

Dosing is highly individualized and must follow a specific titration schedule to minimize the risk of serious rash. For bipolar disorder in patients NOT taking valproate (which inhibits lamotrigine metabolism) or carbamazepine/phenytoin/primidone/rifampin/phenobarbital (which induce lamotrigine metabolism), the recommended titration is:

  • Weeks 1 and 2: 25 mg once daily.
  • Weeks 3 and 4: 50 mg once daily.
  • Week 5: 100 mg once daily.
  • Week 6 and onward: 200 mg once daily (the usual target maintenance dose). Dosage adjustments are required for patients taking concomitant medications that affect glucuronidation. The tablets should be swallowed whole with water, with or without food. The chewable dispersible tablets may be swallowed whole, chewed, or dispersed in a small volume of water.

Precautions

Patients must be informed about the potential for serious, life-threatening rashes, including Stevens-Johnson Syndrome (SJS) and Toxic Epidermal Necrolysis (TEN). The risk is highest during the initial titration period or with a dosage exceeding the recommended titration rate. Any rash, especially one accompanied by fever, blistering, or mucosal involvement, warrants immediate medical evaluation and likely discontinuation of Lamictal. Use with caution in patients with renal or hepatic impairment, as clearance may be reduced, necessitating dosage adjustment. May cause dizziness or drowsiness; patients should be cautioned about operating machinery or driving until they know how the medication affects them. Aseptic meningitis has been reported rarely.

Contraindications

Hypersensitivity to lamotrigine or any component of the formulation. Prior history of Lamictal-associated rash unless the rash was clearly not drug-related.

Possible side effect

Common (>10%): Headache, dizziness, diplopia (double vision), ataxia, nausea, blurred vision, somnolence, rash. Less Common (1-10%): Insomnia, fatigue, rhinitis, pharyngitis, cough, vomiting, dyspepsia, abdominal pain, tremor, anxiety, irritability. Rare but Serious (<1%): Serious dermatological reactions (SJS, TEN, Drug Reaction with Eosinophilia and Systemic Symptoms - DRESS), aseptic meningitis, blood dyscrasias, hepatic dysfunction, multiorgan failure, suicidal thoughts or behaviors.

Drug interaction

  • Valproate (Depakote): Increases lamotrigine levels more than twofold, requiring a significantly reduced Lamictal dose.
  • Carbamazepine, Phenytoin, Phenobarbital, Primidone, Rifampin, Oral Contraceptives containing estrogen: Can significantly decrease lamotrigine levels, requiring an increased Lamictal dose. Note: Dosing may need to be adjusted during the pill-free week of oral contraceptive use.
  • Other CNS depressants (alcohol, benzodiazepines, opioids) may potentiate sedative effects.

Missed dose

If a dose is missed, it should be taken as soon as it is remembered. If it is almost time for the next dose, the missed dose should be skipped, and the regular dosing schedule resumed. Do not double the dose to make up for a missed one.

Overdose

Symptoms of overdose may include increased severity of known adverse reactions such as ataxia, nystagmus, seizures, drowsiness, coma, and intraventricular conduction delay. In massive overdoses, QRS prolongation and arrhythmias have been reported. Management involves general supportive measures and gastric lavage if presented early. Due to its high volume of distribution, lamotrigine is not significantly eliminated by hemodialysis.

Storage

Store at room temperature between 20ยฐC to 25ยฐC (68ยฐF to 77ยฐF). Excursions are permitted between 15ยฐC and 30ยฐC (59ยฐF and 86ยฐF). Keep in the original container or blister pack to protect from moisture and light. Keep out of reach of children and pets.
