Common use

Rhinocort Nasal Spray is primarily indicated for the management of seasonal and perennial allergic rhinitis in adults and children 6 years of age and older. It is also approved for the treatment of non-allergic (vasomotor) rhinitis and for the prevention of the regrowth of nasal polyps post-polypectomy. It is not intended for the relief of immediate, acute symptoms and works best as a prophylactic and maintenance therapy with consistent daily use.

Dosage and direction

Adults and children 12 years and older: The recommended starting dose is 2 sprays (64 mcg) in each nostril once daily, or 1 spray (32 mcg) in each nostril twice daily (e.g., morning and evening). For maintenance, the lowest effective dose should be used. Some patients may be adequately controlled on 1 spray in each nostril once daily.

Children 6 to 11 years: The recommended starting dose is 1 spray (32 mcg) in each nostril once daily. The maximum dose is 2 sprays (64 mcg) in each nostril once daily.

Administration:

  1. Gently shake the bottle before each use.
  2. Remove the dust cap.
  3. Prior to first use, prime the pump by actuating it 8 times until a fine spray appears. If not used for 2 consecutive days, reprime with 1 spray or until a fine mist appears.
  4. Blow nose gently to clear nostrils.
  5. Close one nostril by gently pressing against it with a finger.
  6. Tilt head slightly forward and insert the nasal applicator into the other nostril.
  7. Point the applicator toward the back and outer side of the nose, away from the nasal septum.
  8. While breathing in through the nose, press down firmly on the pump to release the spray.
  9. Breathe out through the mouth.
  10. Repeat the process for the other nostril.
  11. Wipe the applicator with a clean tissue and replace the dust cap.

The therapeutic effect depends on regular use. It may take up to 2 weeks of consistent use to achieve maximum benefit.

Precautions

  • Patients should inform their physician if they have had recent nasal surgery, nasal sores, ulcers, or injury, as healing may be impaired.
  • Use with caution in patients with active or quiescent tuberculous infections, untreated fungal, bacterial, or viral systemic infections, or ocular herpes simplex.
  • Close monitoring is advised for patients switching from systemic corticosteroid therapy to Rhinocort due to the risk of adrenal insufficiency.
  • Although rare, localized fungal infections (e.g., Candida albicans) may occur in the nose and pharynx.
  • Patients with glaucoma or cataracts should be monitored regularly, as intranasal corticosteroids may be associated with the development or worsening of these conditions.
  • The replacement of a systemic corticosteroid with a topical corticosteroid can be accompanied by signs of adrenal insufficiency.

Contraindications

Rhinocort Nasal Spray is contraindicated in patients with a known hypersensitivity to budesonide or any of the excipients in the formulation.

Possible side effect

The majority of adverse reactions are mild and local in nature.

  • Very common (≥1/10): Epistaxis (nosebleeds), nasal irritation (burning, stinging, itching, or dryness).
  • Common (≥1/100 to <1/10): Pharyngitis, coughing, nasal congestion, sneezing, crusting in the nose, headache.
  • Uncommon (≥1/1,000 to <1/100): Ulceration of the nasal mucosa, perforation of the nasal septum (rare, typically associated with improper administration technique aimed at the septum).
  • Rare (≥1/10,000 to <1/1,000): Immediate and delayed hypersensitivity reactions, including rash, contact dermatitis, urticaria, angioedema, and bronchospasm.
  • Systemic effects: Systemic effects of corticosteroids are unlikely at recommended doses but may occur, especially at higher-than-recommended doses over prolonged periods. These may include adrenal suppression, growth retardation in children and adolescents, decreased bone mineral density, cataracts, glaucoma, and psychological effects.

Drug interaction

Formal drug interaction studies have not been performed with Rhinocort Nasal Spray. Budesonide is metabolized via the cytochrome P450 (CYP) 3A4 system. Concomitant administration of potent CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, ritonavir, clarithromycin) may inhibit the metabolism of budesonide and lead to increased systemic exposure and an increased risk of systemic corticosteroid effects. Missed dose

If a dose is missed, it should be taken as soon as remembered. However, if it is almost time for the next scheduled dose, the missed dose should be skipped, and the regular dosing schedule resumed. The patient should not double the dose to make up for a missed one.

Overdose

Acute overdose with Rhinocort Nasal Spray is unlikely due to its topical application and limited systemic bioavailability. Single doses of up to 20 mg of oral budesonide have been tolerated without significant adverse events. Chronic overdose or misuse by significantly exceeding the prescribed dose may lead to the development of systemic glucocorticosteroid effects, such as hypercorticism and adrenal suppression. In cases of chronic overdose, the dose should be reduced gradually under medical supervision.

Storage

  • Store at room temperature between 20°C to 25°C (68°F to 77°F). Excursions permitted between 15°C to 30°C (59°F to 86°F).
  • Do not freeze.
  • Keep the bottle tightly closed and stored in an upright position.
  • Discard the bottle 120 sprays after initial priming, even if it is not completely empty, as the delivered dose may no longer be consistent.
  • Keep out of reach of children and pets.
